At Sunflower Community Supports, we believe every person deserves supports that truly reflect their individual strengths, goals and everyday needs.
A Functional Capacity Assessment (FCA) is a comprehensive assessment completed by one of our experienced Allied Health professionals. It provides a detailed understanding of how a person's disability impacts their daily life and identifies the supports, therapies and funding that may help them achieve greater independence and quality of life.
Whether you're applying for the NDIS, preparing for a plan reassessment, requesting additional funding, or simply wanting to better understand your support needs, our team is here to guide you every step of the way.

Questions, Answered
Do I need an approved NDIS plan to get started?
Not necessarily. Whether you're self-managed, plan-managed or NDIA-managed — or still waiting on your plan — reach out and we'll talk you through your options and what's possible.
Which areas do you service?
We service the northern, western and south eastern suburbs of Melbourne.
Our hubs are located in Epping, Melton and Lakes Entrance.
Are you currently taking on new participants?
Absolutely!
We have openings with support coordinators, speech pathologists, support work, day program, and school readiness program.
Are your support workers qualified and screened?
100%.
We require all our support workers to be qualified and screened before commencing any supports with us.
How do I get started?
We aim to be as accessible as possible. So we have options!
1. Head to our referral tab and complete the intake form for yourself, if you are comfortable doing so.
2. Leave your name and number in the section below "Ready to Get Started" & one of our lovely admin's will contact you via the information provided to walk you through the intake form.
3. Have your support coordinator reach out to us directly.
4. Just give us a call at 0414 416 522 and we can do it on the spot 🙂
