Support Coordination Level 2
Let us support you with implementing your NDIS plan and engage with services that will assist you with achieving your goals. We have vast knowledge of the NDIS and will also support you with ensuring that your NDIS plan reflects your current needs.
Specialist support coordination Level 3
We are experienced with assisting participants with various complex needs and have the clinical skills as well as the know how to assist you with liaising with various services such as housing, family services, judicial system etc to ensure that you are accessing all supports that are available to you, through the NDIS as well as other government services.
Recovery Coaching
At Sunflower we understand that the pathway to recovery is a journey and that we all experience some bumps along the way. We can support you to engage with services at your own pace by spending time getting to know you, linking you in with supports and helping you better understand the NDIS and how it can support you in your recovery.
Speech Pathology
Our speech therapist can assist with developing receptive and expressive communication, language comprehension and social skills. Services are available in the community as well as onsite in Craigieburn clinic.
Occupational Therapy
At Sunflower our Occupational Therapists can support you to reach your goals and maximise your independence. OT can also assist with developing a full functional assessment and recommending appropriate assistive technology and other supports to assist you with improving your day to day and overall well being.
Counselling
We all need someone to talk to from time to time, and whether you have a mental health diagnosis or a different disability it never hurts to have a chat. We offer a space for you to sound board your thoughts without judgement, help you establish your own goals and then assist you to work towards them at your own pace.
Carer Training
If your family member (child or adult) has an NDIS plan but you’re unsure of where to start, we can help train you and point you in the right direction. We can also assist you with finding services and linking in with supports. Additionally we can support you with advise around the services that your family member may require and provide additional education on how to best support your family member.
Support Worker Services
At Sunflower, we ensure that we are here to support you to live an ordinary life both in your own home and when out in the community. We are committed in ensuring that you and those around you are able to build a strong working relationship with workers who support you and that they will assist you to have as much independence as possible in your day to day life whilst catering to your needs.
SIL and STA/ MTA
If you are funded for Supported Independent Living (SIL) supports our experienced support workers can assist you with 24 hour care, either in your home environment or in another setting of your choice. We are also able to provide Short Term Accommodation (STA) and Medium Term Accomodation (MTA) if required. We have a lovely property available in Craigieburn for both STA and MTA and can also support you with accessing this service in other locations as required.
Cleaning and Gardening
At Sunflower. we can support you with keeping your home an yard under control. Our lovely cleaners and gardeners can assist you with keeping your house organised and sparkling and your garden maintained. They also have experience with working with people with disability and will communicate with you to ensure that you are happy with each service that they provide.
